Our administrative team works diligently to confirm eligibility and benefits prior to your visit; however, please note that final coverage determinations are made by your insurance provider.

Any co-pays, deductibles, or non-covered services are the responsibility of the patient and are due at the time of service. For patients without insurance or those seeking services not covered by their plan, affordable self-pay & sliding scale options are available.
